Энтероглюкагон — пептидный гормон из семейства секретина. Второе название глюкагоноподобный пептид-1 (ГПП-1).
Энтероглюкагон (глюкагоноподобный пептид-1) продуцируется L-клетками слизистой оболочки подвздошной и толстой кишок. Энтероглюкагон относится к инкретинам, то есть вырабатывается в ответ на приём пищи. Стимуляторами секреции энтероглюкагона являются триглицериды, а также углеводы химуса. Введённая внутривенно глюкоза не оказывает влияние на секрецию энтеролглюкагона.
Энтероглюкагон (глюкагоноподобный пептид-1) оказывает влияние на многие разнообразные процессы, происходящие в организме. В желудке он ингибирует секрецию соляной кислоты обкладочными клетками, а также ослабляет моторную активность желудка. В поджелудочной железе энтероглюкагон стимулирует продукцию инсулина, тормозит секрецию соматостатина и глюкагона и вызывает иные эффекты. Энтероглюкагон также выступает медиатором ряда процессов в центральной нервной системе, сердечно-сосудистой системе, щитовидной железе, лёгких, почках и других.

Патофизиологический подход в лечении сахарного диабета 2 типа

Стратегический подход к терапии сахарного диабета 2 типа (СД 2) предполагает раннюю и эффективную интервенцию (вмешательство), направленную против хронической гипергликемии. Сегодня ни у кого не вызывает сомнения тот факт, что именно хроническая гипергликемия ответственна за сосудистые осложнения при СД 2. С этой целью многие национальные и международные диабетологические ассоциации определили целевой показатель гликированного гемоглобина (HbA1c) — интегрального показателя состояния углеводного обмена в относительно длительном промежутке времени:
ANAES — Франция: < 6%.
NICE — Великобритания: между 6,5–7%.
ADA/EASD: < 7% в общем для пациентов, < 6% в особых случаях.
IDF: < 6,5%.
Canada: ≤ 7%; ≤ 6% для тех пациентов, которым можно достичь этого безопасно.
Однако данные как зарубежных, так и отечественных исследований свидетельствуют, что лишь около 25% пациентов достигают целевого показателя HbA1c < 7%, предусмотренных консенсусом Американской и Европейской ассоциаций диабетологов (ADA/EASD). И это несмотря на наличие большого арсенала терапевтических средств, направленных на контроль уровня гликемии. Современный взгляд на патофизиологию СД 2 обусловливает такой терапевтический подход, при котором назначается сенситайзер инсулина для борьбы с инсулинорезистентностью и секретоген — для улучшения b-клеточной функции. Однако регуляция гомеостаза глюкозы в организме осуществляется сложной мультигормональной системой, включающей гормоны не только поджелудочной железы, но и гормоны-инкретины, вырабатываемые в кишечнике в ответ на прием пищи.
Доказано, что после приема глюкозы внутрь происходит более выраженное увеличение секреции инсулина по сравнению с тем, которое наблюдается после внутривенной инфузии глюкозы, сопровождающейся идентичным повышением уровня гликемии. Этот эффект, названный «эффектом инкретина», свидетельствует о большой значимости сигналов желудочно-кишечного тракта (ЖКТ) в гормональной регуляции гомеостаза глюкозы.
Пероральная нагрузка глюкозой вызывает гораздо более значительный инсулиновый ответ, чем внутривенное введение глюкозы, вызывающее идентичный профиль глюкозы в плазме крови, поскольку пероральная нагрузка запускает высвобождение инкретинов в разных отделах пищеварительного тракта.
Инкретины относятся к семейству гормонов, стимулирующих секрецию инсулина в ответ на прием пищи. До 70% постпрандиальной секреции инсулина у здоровых людей обусловлено именно эффектом инкретинов, который значительно снижен у больных СД 2 и с нарушением толерантности к глюкозе (НТГ).
Двумя самыми важными гормонами-инкретинами являются:

ГПП-1 принадлежит к суперсемейству пептидных гормонов «глюкагон-секретин», которые объединяет схожесть аминокислотной последовательности с молекулой глюкагона, составляющая от 21% до 48%, чем и обусловлено название «глюкагон-подобный». Несмотря на это, ГПП-1 существенно различается по своему влиянию на основные метаболические процессы. В L-клетках, которые в основном располагаются в дистальной части ЖКТ — подвздошной и толстой кишке, проглюкагон расщепляется не с образованием глюкагона, как в b-клетках, а с отщеплением от С-конца двух глюкагоноподобных пептидов. Эти пептиды, ГПП-1 и ГПП-2, характеризуются практически 50% гомологией по аминокислотной последовательности с глюкагоном.
ГПП-1 имеет короткий период полужизни, который составляет всего 60–90 секунд. Его быстрая инактивация, с образованием неактивных метаболитов, происходит под влиянием дипептидил пептидазы IV (ДПП-IV).
Секреция ГПП-1 дистальными отделами ЖКТ контролируется нервными и эндокринными сигналами, которые инициируются как поступлением пищи в проксимальные отделы ЖКТ, так и прямым воздействием пищевых нутриентов на L-клетки. С этим связан двухфазный механизм секреции ГПП-1. and central GLP-1 sensitive pathways appear to be organised to co-operatively help control food intake and body weight.

Obesity, particularly abdominal obesity, is associated with numerous metabolic abnormalities, including insulin resistance and type 2 diabetes. Obesity, defined by body mass index 30
kg/m2 or greater, is rather weakly associated with increased risk of metabolic derangements, whereas intra-abdominal (mesenteric) fat accumulation is a far stronger predictor of impaired
glycaemic control.1,2 Fortunately, there is good evidence that losing weight is an efficacious means of improving insulin sensitivity and hence decreasing risk of obesity-associated diabetes. Weight loss induced by such diverse means as gastric surgery, pharmacotherapy and lifestyle interventions, including exercise, all result in better glycaemic control and reduced risk
of type 2 diabetes.3–6 In addition to insulin resistance, type 2 diabetes is also associated with impaired β-cell function. Thus the ideal antidiabetic therapy would improve glucose disposal and β-cell function while reducing body weight.
Agonists acting via stimulation of the GLP-1 receptor compose an interesting, innovative class of antihyperglycaemic agents as they address pancreatic function as well as caloric intake.7,8 Other articles in this publication thoroughly review direct and indirect actions of GLP-1 upon β-cell function (see Holst pp. S10–S18 and Gallwitz pp. S19–S25), whereas this article will concentrate on GLP-1 agonist-induced weight loss. Clinical experience obtained
with the use of exenatide demonstrates that a certain subset of subjects with type 2 diabetes is particularly sensitive to GLP-1 agonist-induced weight loss, which appears to be
robustly sustained.9 Conversely, weight loss is not associated with inhibitors of DPP-4. Thus, it appears that higherthan-normal physiological levels of GLP-1 agonists are required to obtain the desired weight loss to overcome this nhibition.
The first hint of a potential role of endogenous GLP-1 in regulation of metabolic control was given by the observation that GLP-1 acts as an ileal brake by reducing gastric motility and hence delivery of calories to absorptive sections of the small intestine.10 In humans, intravenous infusion of GLP-1 was demonstrated to decrease caloric intake and trigger premature satiety and sense of fullness.11 A few years prior to these clinical pharmacological experiments, studies in rodents suggested that the brain is equipped with GLP-1-sensitive sites eliciting negative energy balance.
Conclusion
The endogenous GLP-1 system appears to be highly complex. Several lines of evidence suggest that the peripheral GLP-1-sensitive system feeds into the central GLP-1 pathways, thereby organising the processing mode of the two systems in series rather than in parallel. Both as a hormone and as a neurotransmitter, GLP-1 has a positive impact on energy homeostasis that leads to weight loss and improved metabolic control. This knowledge can be exploited therapeutically for both obese people and subjects with type 2 diabetes. At present, the GLP-1 agonists seem to be a desirable option for treatment of type 2 diabetes, as they improve glycaemic control, improve pancreatic function and induce clinically meaningful weight loss. Taken together, GLP-1 agonists have the potential to modify type 2 diabetes disease progression
